Silver Diamine Fluoride (SDF) is a non-invasive treatment option for cavities (caries), using a topical medicine that is painted on the tooth, to prevent them from increasing in size and spreading. It can be used as an alternative to a more invasive surgical (drill and fill) approach.
What is SDF?
Silver Diamine Fluoride is a liquid containing:
Silver
Fluoride
Water
Ammonia
The silver in the formula kills the bacteria that causes cavities, thus preventing further growth the cavities. The fluoride aids in remineralization of affected teeth and also prevents the development of new cavities.
It was first used in Japan close to 100 years ago and has recently gained world wide popularity in paediatric dentistry.
The advantages of using SDF
It is a quick easy and painless procedure that can be used to avoid or delay more invasive drill and fill procedures
It is a great treatment option for nervous individuals
Relieves sensitivity
Remineralizes your natural tooth structure
Prevents biofilm adhesion reducing plaque formation
Arrests 80% of cavities if applied twice yearly
The disadvantages of using SDF
Fortunately, SDF has very little side effects and is proved to be safe for use in children. Research shows that it is a great tool in the management of dental caries.
The disadvantages of using SDF are:
It permanently stains cavities black, although this can be masked with a tooth coloured dental filling material.
Cavities (holes) that trap food may still require a restoration
Not an option for deep cavities that penetrate the pulp (nerve) of the tooth.

The SMART Technique
Silver Modified Atraumatic Restorative Treatment (SMART) is a minimally invasive treatment option for cavities treated by Silver Diamine Fluoride (SDF) using Glass Ionomer Cement (GIC)
Glass Ionomer cement is a biocompatible restorative material that binds chemically to the tooth. It can be placed without drilling over the SDF treated cavities to:
Mask the black discolouration of the tooth
Prevent food impaction in cavities
Prevent reactivation of the bacteria in the cavities from sugar in the diet.
This technique is ideal in younger patients and in nervous patients requiring dental treatment.
